How Laparoscopic Hysterectomy Works 

In previous blogs, I’ve explained that laparoscopic surgery involves making 2 to 3 small incisions in the abdomen. Through one of these incisions, we insert a telescope or camera connected to a high-definition monitor. This allows us to view the internal organs on a screen. The other incisions are used to insert long-handled instruments, such as scissors or forceps, which help us perform the surgery. 

The Procedure Explained 

The uterus, fallopian tubes, and ovaries are connected to the abdominal wall by ligaments. Using long-handled instruments, we carefully cauterize and cut these ligaments and blood vessels to free the uterus, tubes, and ovaries from their attachments. Once everything is separated, we make a small incision in the vaginal canal to remove the uterus.

The vaginal canal, through which a baby is delivered, can also accommodate the removal of a large uterus. If a baby weighing 3.5 kilograms can be delivered through this route, a large uterus can also be removed in the same manner.

Handling Large or Irregular Uterus

In some cases, particularly if the uterus is irregular due to multiple or very large fibroids, we might encounter difficulty removing the entire uterus in one piece. For such situations, we use a technique known as the “ribbon technique.” This involves twisting and cutting the uterus into smaller pieces, which are then removed using a ribbon-like strip. This method ensures that the entire uterus is removed through the vaginal canal, leaving no residual tissue behind.
